LPN Intake Coordinator – Full-Time

Location: Maitland, FL | 🕒 Schedule: 40 hours/week, On-Call Rotation

What You’ll Do

As the LPN Intake Coordinator, you’ll be the heartbeat of our office operations and intake process. Responsibilities include:

• Answering phones and managing fax communications

• Entering patient referrals into the EMR system

• Assisting with coordination of home health services

• Creating, modifying, and maintaining forms and records

• Filing, distributing Medicare paperwork, and tracking physician orders

• Assembling admission packets

• Supporting quality assurance and regulatory compliance efforts

• Jumping in to help your team whenever needed

Qualifications

• Active LPN license (required)

• Valid driver’s license & reliable transportation

• 1+ year of home health experience (required)

• Experience scheduling home health referrals (preferred)

• Proficiency with Microsoft Office Suite (Word, Excel, etc.)

• Ability to participate in on-call rotation
