1. Make contact with nursing homes and hospital discharge coordinators inRegion 1B under established guidelines and time frames.
2. Coordinate with discharge planners, potential candidates and their allies on
transition and admission into the MI Choice Program.
3. Perform “Imminent Risk of Nursing Facility Placement Assessment” form to
determine if candidates qualify for diversion.
4. Conduct the Michigan Department of health and Human Resources (MDHHS)
Telephone Intake (MIG) in Compass to determine if a candidate qualifies for a
face to face Nursing Facility Level of Care Determination (NFLOC/ LOCD).
5. Meet face to face with the candidate and allies to complete a NFLOC/ LOCD
with Freedom of Choice which determines program eligibility. Enter the
completed NFLOC/ LOCD into the State of Michigan electronic data system on
the start of service date.
6. Complete documentation and follow-up in compliance with all Rules,
Regulations, Laws, Standards, and Policies that govern the program.
7. In coordination with the participant, develop a Care Plan, within required
timelines from the initial assessment.
8. Prepare service authorizations after the initial assessment.
9. Complete the Program Enrollment Type (PET) and submit to Office staff on
the service authorization start date.
10. Assist the participant and allies to maximize their understanding, involvement
and responsibility regarding the authorization of services.
11. Provide education and guidance to encourage family empowerment, social
skill development, and relationship building. Educates individuals/families and
provides resources for Self Determination options
12. Provide training and guidance to the family specific to meeting the needs and
wishes identified in the child and family’s service plan.
13. All customer related services and reports are delivered within specific
timeframes as established by standards of practice.
Must be a Licensed Bachelor of Social Work (LBSW) OR Limited License Bachelor of
Social Work (LLBSW)
