About the Role
The Intake Clinician, under the guidance of Team Leaders, Managers, and the Community Clinical Director, collaborates closely with the Intake Registrar, partner Intake and Assessment Clinician, and multidisciplinary teams across the Mental Health Service to coordinate referrals and assessments.

  • Manage the Intake and Assessment position duties in collaboration with the partner Intake and Assessment clinician.
  • Collate, analyse, and review referral information from internal and external sources, and proactively obtain additional information where required
  • Maintain accurate and up-to-date records by reviewing and updating referrals, assessments using relevant systems, including CMI, PAS, MRO, and SharePoint.


Classifications for this position will range from RPN Grade 4 NP75 - NP77 ($121,274 - $130,603 per annum pro rata)



Your Contribution

  • A minimum of at least 5 years post qualification clinical experience within Mental Health for SW, OT and RPN
  • Registration with Australian Health Practitioner Regulation Agency (all disciplines except Social Work)
  • Relevant postgraduate qualification in mental health for nursing staff
  • Occupational Therapists - Registration with the Occupational Therapy Board of Australia
  • Social Workers - An approved degree in Social Work (by the AASW) and eligibility for full membership of the Australian Association of Social Workers.
  • Current Victorian driver’s license
  • A valid Nationally Coordinated Criminal history check, or willing to obtain

About St Vincents Mental Health.

While the role is located at Hawthorn Community Mental Health Service, it will work collaboratively with its counterpart positions at Clarendon Community Mental Health Service and across the SVHM mental health service.

St. Vincent’s Mental Health (SVMH) includes an adult area mental health service for the inner-city areas of Yarra and Boroondara. The Adult Mental Health service components are: The Footbridge Community Care Unit (CCU), a 44 bed Acute Inpatient Service (AIS) located on the St. Vincent’s Hospital Fitzroy Campus, the North Fitzroy PARC and two community

mental health services (CMHS) at Hawthorn and East Melbourne (Clarendon CMHS) consisting of Continuing Care (CCT), Mobile Support and Treatment Service (MSTS) and Homeless Outreach (CHOPS) services, Psychiatric Triage and Crisis Assessment and Treatment (CAT) service, Police, Ambulance and Clinical Response (PACER), ED Mental Health, Extended Triage and The Primary Intervention Care Team (PICT), Hospital Outreach Post Suicide Engagement (HOPE), Supporting Tenants at Yarra Housing Project (STAY).
