Intake Admissions Specialist - Home Health
Schedule: Monday - Friday 8:00am - 5:00pm with weekend rotation
YOU MUST LIVE IN THE DETROIT/TROY AREA FOR THIS POSITION AS THE TRAINING AND OTHER RESPONSILBILITES REQUIRE YOU TO BE IN OFFICE, BUT THE MAJORITY OF THIS ROLE WILL BE REMOTE.

The Intake Admission Specialist, Community will communicate with referral sources, staff, and community resources in transitioning patients to begin receiving home care services. The Intake Admission Specialist, CommunityrepresentsASHN and builds positive working relationships. The Intake Admission Specialist, Community is a knowledgeable resource for patients and staff members. Where necessary, the role will collaborate with other departments andreferringinstitutional and community providers toassistwith the retrieval of orders from physicians in the communityin a timely manner
KEY RESPONSIBILITIES
Processes all daily patient referral and intake operations including all intake processes and policies.
Maintains comprehensive working knowledge of contractual relationships and ensures that patients are admitted according to contract providers.
Communicates with multiple service providers to coordinate care.
Follows up with patientsin regards toStart of Care (SOC) & services being provided to measure satisfaction.
Responsible for referral related phone calls.
Calls physicianto obtain orders when needed.
Follows up andreportsdelay stages, non-admits, and any other SOC issues.
Establishes andmaintainspositive working relationships with current and potential referral sources.
Builds and monitors community and customerperceptionsof the Company as ahigh qualityprovider of services.
Maintains comprehensive working knowledge of community resources andassistsreferral sources in accessing community resources should services not be provided by the Company.
Provides communication and support to Community Educators and or CommunityLiaisonsreferral sources and clients.
Understands and practices agency policies and procedures.
Stayscurrent with CMS guidelines.
Performs workflow and data reportingin accordance withCMS guidelines.
Ensures compliance with all state and federal referral/intake regulatory requirements.
Answersoverflowphone calls.
Maintains incoming faxes via fax server.
Arrives at assigned location on scheduled workday. Works according to designated hours.
Accurate and efficient data entry.
Ability to communicate using voice and email in building working relationships with patients and employees.
Performs duties consistently with the use of a computer, fax,copierand phone.
Completes andsubmitsall required documentation within specified company requirements.
Dexterity & vision to complete documentation on a computer.
Obtain orders from physicians in the community.
Other duties as assigned
QUALIFICATIONS
Familiar with medical terminology.
Knowledge of governmental regulations and private payer practices.
Ability to create positive impressions and communicate with a variety of people.
Ability to market aggressively and deal tactfully with customers and the community.
Personable and courteous working relationships.

EDUCATION AND CREDENTIALS
Associates degree and/or two years of experience in the healthcare field with one year in homecare;or combination thereof.
