This faculty position in the Department of Clinical Pharmacy partners with University of Colorado Medicine (CU Medicine) to support system-level population health and quality improvement initiatives. CU Medicine is the region’s largest multispecialty physician group practice, providing business operations, revenue cycle, and administrative services to support more than 3,500 University of Colorado School of Medicine physicians and advanced practice providers. Within CU Medicine, the Office of Value-Based Performance (OVBP) oversees value-based programs, including business operations, quality improvement, innovation, and program evaluation.

This is a part-time role at 30% FTE, with the faculty member serving within OVBP under the Director of Population Health Pharmacy as part of a multidisciplinary team. The team operates from a strategic perspective to design and implement innovative programs and processes that improve healthcare quality across University of Colorado School of Medicine–affiliated clinics. The position focuses on advancing value-based care initiatives that enhance quality and overall healthcare system value, while also supporting efforts involving clinical pharmacist faculty who provide direct patient care in family medicine, internal medicine, and other ambulatory care settings across CU Medicine and UCHealth.

Key Responsibilities:

·Provide centralized population health outreach to address care gaps important to value-based programs, potentially including but not limited to:

oMedication Adherence program

oStatin use in people with diabetes

oOsteoporosis management in women who had a fracture

oFlu vaccine

oCare for Older Adults - Medication review

oMedication cost-reduction opportunities

·Analyze population data to identify value opportunities related to cost or quality.

·Assist the Director of Population Health Pharmacy with support of execution and evaluation of existing decentralized population health efforts by the CU Primary Care Pharmacist Team, potentially including but not limited to:

oPrograms developed in collaboration with the Ambulatory Health Promotions team (e.g., Fracture Liaison Service, hypertension pharmacist outreach initiatives, gout medication optimization)

oMedicaid home BP monitor loaner program

·Performs drug prior-authorization reviews for the CU Anthem Exclusive Health Plan using Anthem high-cost drug guidelines and subsequently sends denial recommendations to the Utilization Management Medical Director for final review.

Work Location:

Hybrid – this role is eligible for a hybrid schedule of # days per week on campus and as needed for in-person meetings.

Qualifications:

Minimum Qualifications:

Applicants must meet minimum qualifications at the time of hire.

  • Applicants must have a PharmD degree

  • Completed an ASHP-accredited PGY1 residency training

  • Candidates must have or be eligible for pharmacist licensure in Colorado.

Preferred Qualifications:

  • Previous experience in population health, managed care, or other experience with healthcare quality programs

  • Previous experience in drug prior-authorization reviews
