Under supervision, the Medication Reconciliation Pharmacist, when assigned to a specific patient population, shall be responsible for the medication reconciliation process at the patient’s admission (and/or discharge if assigned) to prevent medication errors at this patient transition point. The pharmacist shall, by interviewing patients and working closely with pharmacy technicians, create a complete and accurate list of all pre-admission medications. The pharmacist shall compare compiled medication lists against provider orders to identify discrepancies and, if appropriate, make changes with provider permission.
