
Essential Job Duties:
Participates and assists in data collection in a systematic and organized manner to provide high quality and safe outcomes.
Contributes collected data to the development of a plan of care in collaboration with other healthcare professionals that contributes to high quality and safe outcomes.
Implements a plan of care with measurable criteria consistent with world class performance levels.
Collaborates with other team members to evaluate the plan of care with the patient and family/support system to maintain world class levels of patient satisfaction and outcomes.
Contributes to a high-performance work environment by seeking educational resources that enhance and maintain current knowledge and skills.

Minimum Qualifications:
Education -- Graduate from an accredited school of practical nursing. Practical Nurse license from the Indiana State Board of Nursing. Some area may require a current and valid driver’s license.
Experience -- Graduate of LPN program.
Reports to:The Registered Nurse and is accountable to the Nurse Manager.
