Inpatient Case Manager MSW - Complex Cases

40 hours/week
Hours
Monday – Friday: 8:30am -5pm, along with a weekend rotation of approximately every 4 weeks, and holiday and on-call commitments.
Responsible for the provision of medical social work services to patients receiving care in the hospital setting. Assesses the social, cultural, environmental, and financial situations, as well as the disposition needs of each referred patient. Collaborates with healthcare team members for the patient and community organizations to identify needs and resources in the hospital and community. In consultation with the healthcare team develops effective transition plans. Collaborates with the RN Case Manager to ensure a smooth flow of patients through their hospital stay.
Education
Master's Level Degree in Social Work
Certification/Licensure
Certification in Case Management (CCM, CMAC or ACM-SW) is required within one year of eligibility
BLS is required within 90 days of hire
Experience
1 year of related acute care and/or healthcare experience required, and is defined below:
Educational internship experience in an acute care facility can be considered
OR
Healthcare experience refers to the care delivered in one of the following settings: hospital or an acute care facility
