Monitors, coordinates and evaluates the care and appropriateness of hospitalization; initiates referrals. Participates in development, implementation and analysis of care delivery standards and outcome measures.

Essential Duties and Responsibilities:

  • Completes Continuity of Care assessments and establishes appropriate plans by initiation of timely interventions through collaboration with patients, families and multidisciplinary care team respecting choice of patient
  • Concurrently reviews clinical documentation in medical record to assure timely progression of care transitions
  • Collaborates with multi-disciplinary team and informs them of variance from established care pathways to ensure timely and appropriate progression of care and eliminates barriers for next site of care transitions
  • Assists in expediting care delivery to avoid delays in timely service provision
  • Provides feedback to medical staff and conduct education as expert resource regarding care coordination, discharge planning needs and regulatory requirements
  • Delivers CMS regulatory notices within CMS established timeframes
  • Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits
  • Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct

Required Qualifications

Education and Training

  • Currently licensed as a Registered Nurse in the state of Illinois.
  • BSN preferred. 2-5 years previous Case Management experience preferred.
  • Current CPR
  • Relevant hospital nursing; hospital case management; insurance case management experience preferred

Work Shift Details:

Days, Two-day shifts in a six-week schedule period with one weekend shift per 6-week schedule; one recognized holiday shift per year

The final pay rate offered may be more than the posted range based on several factors including but not limited to: licensure, certifications, work experience, education, knowledge, demonstrated abilities, internal equity, market data, and more.

The expected pay for this position is listed below:
