DUTIES & RESPONSIBILITIES:
Responsible for all aspects of assigned caseload, including meeting with clients living in Community Residence as needed but at least weekly. Make referrals to medical, mental health and/or substance abuse services as needed, and continue to involve providers in the clients care. Provide clients with supportive counseling. Make regular visits to client room, and monitor clients to ensure a safe and secure environment. Teach and assist clients with activities of daily living. Complete client 2010 e housing applications, service plans, and psychosocial reports. Provide appropriate level of support including escorts if necessary for client. Ensure that all clinical files and reports are complete and up to date. Assist with intake and admission process. Participate in community meetings. Provide emergency first aid/CPR assistance when needed. Related duties as assigned.
HOURS:
Full-time, 37.5 hours per week
Sunday 3pm-11:30pm; Monday/Thursday 1pm-9:30pm; Tuesday-Wednesday 7am-3:30pm
QUALIFICATIONS:
BA preferred. HS diploma/ GED required and significant experience working with the substance abuse and/or mentally ill population required. Excellent writing skills. Basic computer literacy required. CPR training certification or willingness to take training class in CPR.
