OF POSITION

Provides care coordination activities for clients referred from the Alton CCBHC clinic. Engages clients, coordinating follow-up activities, and maintaining knowledge of transition requirements for individuals' post-hospitalization or care transitions. Requires collaboration with leadership to ensure high-quality service delivery and positive outcomes, while also utilizing personal lived experience to support clients effectively.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Establishes relationships and practice core coaching skills to transform client values/desires into action.
  • Shares personal recovery experiences and develop authentic peer-to-peer relationships.
  • Coordinates with cross-functional teams to ensure engagement and service activities are completed for clients.
  • Works in consultation with clients, family members, and other providers to link clients with necessary resources.
  • Represents Centerstone to internal and external stakeholders and create relationships to develop CCBHC clinics.
  • Assists clients in overcoming barriers to goals, including access to care and navigating health care systems.
  • Utilizes evidence-based methodologies to provide excellent clinical services.
  • Navigates crisis situations to ensure the safety and stabilization of clients.

KNOWLEDGE, SKILLS & ABILITIES

  • Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) for effective document creation and data management.
  • Strong written and verbal communication skills for clear interaction with colleagues and clients.
  • Excellent customer service skills to enhance client satisfaction and engagement.
  • Knowledge of community resources and referral procedures to assist clients appropriately.
  • Understanding of confidentiality guidelines to protect sensitive information.
  • Strong interpersonal skills to foster positive relationships with team members and community partners.
  • Familiarity with virtual communication tools to facilitate remote collaboration.

QUALIFICATIONS

Education Level

Associate’s Degree required. HS Diploma/GED with five years of lived experience may be accepted with Director approval.

Experience in personal recovery process and must be able to share recovery journey.

Years of Experience

Must be at least 21 years of age.

Experience in the behavioral health field is preferred.

Certification/Licensure

Must successfully complete all required background screenings.

Ability and willingness to obtain credentialing if required.

$18.20--$25.50
