Nurse Practitioner – Hybrid Primary Care (Home Visits + Clinic) | Williamsburg, NY
Outpatient practice in Brooklyn is seeking a full-time Nurse Practitioner to provide a mix of home-based and office-based primary care in Bedford-Stuyvesant. This role is ideal for a provider who enjoys relationship-driven medicine and delivering care directly in the community.
The Nurse Practitioner will provide comprehensive primary care across two care settings: in-office visits and scheduled home visits. The schedule is structured around approximately 2–3 days of home visits and 2–3 days in the clinic.
You’ll manage chronic conditions, conduct wellness visits, perform routine follow-ups, and support preventative care in a fully outpatient environment. The practice offers strong administrative support and a steady patient base.
Nurse Practitioner (FNP, AGNP, or similar)
Active New York NP license
Outpatient primary care experience preferred
Comfortable with hybrid care model (home visits + clinic)
Strong communication and autonomy
EMR proficiency
Competitive base salary $150k+ depending on experience
