Specific Responsibilities:
This is a Human Services Program Specialist position in the Florida Department of Health, Palm Beach County Health Department (CHD), HIV/AIDS Program, AIDS Drug Assistance Program (ADAP). Work is performed under the supervision of the Public Health Services Manager A-SES. This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions. Responsible for the day-to-day operations of the Ryan White Part B-funded AIDS Drug Assistance Program (ADAP), which includes performing work in accordance with the ADAP Policy Manual, enrolling clients into ADAP, determining eligibility, for Part B and ADAP only, for new and existing clients, and provides education, counseling, and navigation services to ADAP clients.
This position is designated as a sensitive position and is required to maintain confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) Information Security Policy, Protocols and Procedures. This position may view, update, or release information. Has access to the following confidential data sets: Clinical Information, HIV/AIDS Case Reporting and Surveillance, and Administrative. Has Health Management System (HMS) access for the purpose of entering Employee Activities Record (EARS), and HMS to verify Labs and Prescription. Has access to Change Health Care (CHC) to verify Medicaid and or Medically Needy, and access to Provide Enterprise (PE) to performeligibility and enter ADAP required data. Utilizes several components of the HMS, including the Electronic Health Record (EHR), and PE to facilitate patient activity and determine eligibility for ADAP. Utilizes the statewide ADAP system to perform all ADAP client and programmatic functions.
Completes new client enrollments and annual re-enrollments. Updates client information in the statewide ADAP system. Conducts interviews to determine eligibility for ADAP and other patient care services. Tracks client eligibility, re-determination dates, and adherence. Provides technical assistance to community partners and stakeholderson ADAP policies and procedures and eligibility requirements ensuring community awareness of ADAP program and maximized usage. Provides secondary prevention information as needed. Provides HIV post-test counseling, including interpretation of HIV test results, and education on HIV treatment options, as needed and as requested by the client. Assists clients with learning how to navigate the HIV patient care system, including ADAP, eligibility, labs, treatment adherence, case management and appointments with providers, nurses, and mental health. Translates medical information for clients, assesses clients’ support systems, and provides referrals and/or linkages to other community services, including but not limited to, social security, community pharmacy, and private providers. Securely communicates with case managers through Provide Enterprise parts A & B and processes referrals sent through Provide and faxed from CHD and community partner agencies.
Completes eligibility assessments, for Part B and ADAP only, for new and existing clients.
Provides referrals to clients who do not qualify for ADAP to community partners. Verifies client insurance, e.g. Medicaid, Medically Needy, Medicare, HMO or other insurance. Ensures lab order and client records are kept updated and notifies clients of expiring labs and prescriptions. Creates web user accounts for clients in the ADAP system making it easy for clients who cannot come into the CHD are able to submit the required eligibility documentation.
Reviews clients flagged by HIV Section who may be eligible for marketplace insurance, to determine eligibility. Communicates with client’s individual case managers at Ryan White funded community partner agencies that those clients are eligible to be enrolled in the marketplace. Open enrollment reviews the existing clients who are currently enrolled in a marketplace plan to determine if their current plan is most suitable for their needs as an ADAP client. Contacts clients who are not in case management schedules an appointment with a local provider Affordable Care Act (ACA) Navigator and assists them with signing up for the marketplace. Scans and uploads the final plan into each client profile in Provide Enterprise. Follows up with the contracted Insurance Benefit Management agency to ensure payments are made. Refers clients to Ryan White Part A community partner agency for marketplace payment if clients are not eligible for premium payment by ADAP.
Issues client medications at the FDOH ADAP office sites and maintains an ongoing inventory system, including receiving, storing and returning of medications, pursuant to State regulations. Distributes patient drug information sheets with medications. For uninsured clients, communicates with local pharmacies registered through the Pharmacy Benefits Manager (PBM) to enroll clients and request meds on behalf of clients. For insured clients, communicates with any local pharmacy of clients’ choosing to request meds on behalf of clients.Ensures all required documents are scanned into PE at enrollment and as required for audit purposes. Processes clients efficiently and timely to ensure a consistent flow. Schedules returning appointments. Reports immediately to ADAP Supervisor, or designee, any client problems or complaints. Ensures ADAP supervisor is informed of all ADAP-drug-related issues in Palm Beach County. Assists other ADAP staff in resolving ADAP issues/problems/questions, as needed. Responsible for inventories, and orders supplies for all of the ADAP Program sites.
Administers client surveys, as needed and as requested, by the State or by local Program. Responds to requests for technical assistance. Coordinates and manages the ADAP office ensuring office coverage, and any other activity needed to maintain office operations. Coordinates in-services and trainings for CHD staff, community partners, and stakeholders, as needed. Attends training, educational courses/seminars, meetings and in-services as needed. Participates in scheduled clinic team meetings, e.g. quality assurance (QA) record audits, and general staff meetings, and in-services.
