Job Description

Compassionate Human Services Practitioner with expertise in psychosocial assessment, case management, HIV/AIDS education, and care coordination for adolescents and young adults. Skilled in conducting comprehensive evaluations, connecting clients with community resources, navigating healthcare and benefit systems, and collaborating with multidisciplinary teams to improve health outcomes and access to care.

This position is grant funded.

This position required a Level II Background check.

  • Responsible for assessing psychosocial needs of adolescents and young adults served in the USF Pediatric Program.
  • Provide educational support services and counseling to patients and family.
  • Mentors and support groups regarding care and preventative measures to be taken to guard against infection.
  • Serve as a Liaison in the coordination of services for affected youth requiring community resources.
  • Serve as initial intake person for newly diagnosed young adults and secure eligibility documentation.
  • Perform comprehensive psychosocial evaluations on new patients and annual follow up evaluations.
  • Serve as liaison with community, outreach, and testing agencies to bring newly identified youth into care.
  • Serve as designated liaison between the clinical team and families.
  • Outline mental health concerns related to the diagnosis.
  • Manage the development, implementation, coordination, and evaluation of HIV education programs for the adolescent HIV program.
  • Provide developmental assessment tools and curriculum for patient population conducting the MINI and CRAFT screenings.
  • Assist clients with obtaining funding for medical care, applying for Medicaid, negotiating ADAP, and other resources.
  • Assess client needs and collaborate with community resources for client referrals.
  • Serve as a liaison for those seeking financial and legal counseling during the referral process.
  • Provide education and assist youth in accessing community resources (community case management).
  • Provide education and assist youth with navigating health care systems (Medicaid, Medicare, share of cost, private insurance, AIDS Insurance Continuation Program, COBRA).
  • Identify alternative funding sources and educate/ assist youth with process (RW, ADAP).
  • Identify and educate youth on accessing transportation for appointments.
  • Collaborate with physicians, nursing coordinators, community-based managers, CMS, child life therapist, psychologist, psychiatrist, and Chaplin/ clergy services to develop medical education and assistance for HIV/ AIDS patients.
  • Research duties related to the Adolescent Trial Network and communicated and coordinate with Hillsborough County School System and community groups to implement comprehensive sexual health education.
  • Communicate and collaborate with Tampa Housing Authority serving as liaison between THA and USF Peds ID.
  • Provide assistance to clinical team and perform other duties as needed.

Minimum Qualifications

  • Each position assigned to this classification must require specific training at the Bachelor’s degree or Master’s degree level in Education, Special Education, Psychology, Social Work, or other field appropriate to the program content.
  • Bachelor’s degree candidates must have at least two years of applicable experience.
  • The Master’s degree may substitute for the required experience. Specific certifications or licensures may be required or preferred for certain program areas.

SB 1310: Substitution of Work Experience for Postsecondary Education Requirements

A public employer may include a postsecondary degree as a baseline requirement only as an alternative to the number of years of direct experience required, not to exceed:

  • (a) Two years of direct experience for an associate degree;
  • (b) Four years of direct experience for a bachelor’s degree;
  • (c) Six years of direct experience for a master’s degree;
  • (d) Seven years of direct experience for a professional degree; or
  • (e) Nine years of direct experience for a doctoral degree
  • Related work experience may not substitute for any required licensure, certification, or registration required for the position of employment as established by the public employer and indicated in the advertised description of the position of employment.
  • Minimum Qualifications that require a high school diploma are exempt from SB 1310.
