Job Description

Title: Home to Stay Direct Support Professional

Reports to Home to Stay Program Supervisor

Job Objectives:

Under the direction of the Home to Stay (HTS) Program Supervisor, assist with the provision of residential services to 12 individuals enrolled in HTS; a supported housing program providing intensive daily supports on site and in the community.

Provide Trauma Informed Care to all program participants, while utilizing a strength based approach and person centered model.

Perform duties while actively promoting an environment consistent with MHA’s Cultural Equity Policy and WELCOME Orange.

Responsibilities:

· Establish and maintain good rapport and working relations with all program participants.

· Establish and maintain positive and productive external relationships with providers, family members and others involved in the program participant's treatment.

· Take clients on social outings and have scheduled trips to various parks, shopping stores, grocery shopping every weekend.

· Instruct, assist and supervise program participants in the skills of daily living including, but not limited: to house maintenance tasks (cleaning their apartments), medication disbursement, financial management, personal hygiene, health care, social/recreational activities, laundry, and grocery shopping.

· Maintain program participant charts and data entry into MHA’s Electronic Record System - Foothold.

· Encourage the development of responsible decision making skills.

· Contribute to and participate in the development and implementation of individual service plans.

· Intervene in crisis situations according to agency policies and report all information in an objective and professional manner.

· Record all pertinent data relative to program participants and the operation of the program in daily log book.

· Protect confidentiality and ensure program participants’ rights.

· Maintain Valid American Red Cross / American Heart Association CPR / First Aid certification.

· Develop and Maintain relationships with landlords on behalf of the clients and agency

· Monitor self-medication of program participants and document it accurately.

· Provide advocacy and act as liaison with other service providers in the community.

· Ensure program participants are transported to needed services.

· Work with individuals and groups to facilitate social/recreational activities, which promote rehabilitation and recovery.

· Participate in training sessions designed to enhance growth and skill development.

· Assume necessary functions of program as assigned by the Assistant Director during any absence of HTS Program Supervisor.

Requirements

Qualifications:

· High School Diploma.

· 1 year experience working with individuals with a mental illness or individuals that are seriously emotionally disturbed.

· Ability to work independently with minimal direct supervision.

· Effective written and oral communication skills.

· Must be organized, self-motivated and have the ability to coordinate multiple tasks simultaneously.

· Ability to exercise sound judgment under crisis situations and to abide by regulations regarding confidentiality.

· Valid NYS Driver’s License and ability to drive with current driving record acceptable for agency insurance coverage.

Terms of employment/benefits:

· Non-Exempt.

  • Full Time Position Sunday – Thursday 9:30-5:30 pm

· Paid time off, holidays and sick leave according to MHA policy.
