The Health Promotion Disease Prevention Technician (HPDP Technician) supports the development, implementation, and evaluation of health education and disease prevention initiatives to improve community health and well-being. This position assists in organizing events, conducting outreach, providing culturally appropriate education, and promoting healthy behaviors aligned with Sage Memorial Hospital’s Mission, Vision, and Core Values. Assists in delivering evidence-based health promotion programs (e.g., nutrition, physical activity, chronic disease prevention, injury prevention) through activities and health education. Distributes culturally tailored health materials and resources. Collaborate with the Community Health Division departments and other Sage Memorial Hospital or outside entities on planning and integrated service delivery. Ensures quality patient care delivery and maintains confidentiality of all privileged information.

Essential Duties & Responsibilities:

  • Provide standardized, appropriate educational material and learning opportunities to the community members in various settings
  • Coordinate and plan health promotion and disease prevention activities at SMH, the Mobile Unit, and in the community
  • Conduct community outreach and education in schools, chapter houses, senior centers, wellness centers, other community locations, and at local events. Ensures data is gathered and accurately documented, demonstrating the utilization of the appropriate process to promote patient outcomes.
  • Assists with planning and implementing workshops, health fairs, and wellness activities and events. Supports data collection, program evaluation, and report writing. Work closely with professional providers and other clinic staff to plan and evaluate patient needs and care.
  • Assists with scheduling, logistics, and documentation of outreach events. Participate in community education events at the chapter and hospital levels.
  • Provides health information and referrals to appropriate clinical or public health services.
  • Maintains accurate records of community contacts and services provided.
  • May cross-train to assist other Community Health departments in providing integrated service delivery.
  • Initiates and maintains professional relationships with external resources and facilities to establish efficient patient care delivery.
  • Performs other related duties as assigned.



Qualifications

Minimum

Qualifications:

  • Associate’s Degree in Health Promotion, Public Health, Nutrition, or a related field.
  • Must possess at least two (2) years of progressive work experience in community education, health education, disease prevention, or related field.
  • Must possess a current American Heart Association (AHA) Basic Life Support (BLS) certification.
  • Must maintain a valid, unrestricted Driver’s License.
  • Must successfully pass the Employee Health requirements (mask fit, immunizations), and Pre-Employment requirements (background investigation, drug screen).

Knowledge, Abilities, Skills, and Certifications:

  • Professional knowledge and skill with patient or community education (role as a health educator or Medical Assistant experience)
  • Ability to communicate to small or large groups
  • Ability to identify and utilize evidence-based educational material
  • Ability to investigate and analyze information and to draw conclusions.
  • Ability to design, develop, deliver, and evaluate educational programs and materials.
  • Ability to provide technical advice, guidance, and support to colleagues and professional staff in area of specialty.
  • Knowledge of effective principles and practices of patient education and development.
  • Ability to communicate effectively in the English language, both verbally and in writing, with staff and the public.
  • Ability to establish and maintain professional relationships with individuals of varying social and cultural backgrounds and with co-workers at all levels.
  • Ability to work effectively with individuals and demonstrate team-building skills with empathy and enthusiasm.
  • Ability to manage multiple concurrent projects.
  • Ability to react calmly and effectively in emergency situations.
  • Ability to maintain strict confidentiality.
  • Ability to speak the Navajo language and/or familiarity with the Navajo Way, preferred.
  • Ability to work independently and meet strict timelines.
  • Ability to demonstrate excellence and continually seek improvement in results.
  • Skill in operating business computers and office machines, including in a Windows environment, specifically Word, Excel, Access, and presentation software (such as PowerPoint).
  • Ability to think critically, analyze problems, project consequences, identify solutions, and implement recommendations.

Physical Demands:

While performing the duties of this job, the employee is regularly required to sit during office duties and walk during outdoor/community events; use hands to handle or feel; reach with hands and arms; and talk and hear. The employee is frequently required to walk and stand, as well as stoop, kneel, and crouch. The employee must occasionally lift and/or move up to 50 pounds.

Work Environment:

Work is generally performed in an administrative setting with a moderate noise level. Employee may be exposed to potentially infectious bodily fluids. Extended hours and irregular shifts may be required. Occasional home visits or visits to the mobile unit may be required.
