Job Description

GENERAL DESCRIPTION OF WORK

The HOA Housing Stabilization Specialist is responsible for providing housing-focused support to older adults experiencing housing instability. This role involves connecting individuals with trauma-informed, culturally and age-appropriate services tailored to their unique needs. Duties include assessing housing and service needs, arranging, coordinating, and monitoring the delivery of individualized services to meet participants' needs. The Specialist may also provide crisis counseling, linkages to medical care, and assistance in overcoming barriers to housing stability. Primarily working daytime hours Monday through Friday, the position may require occasional evening or weekend adjustments with supervisory approval. The Housing Stabilization Specialist reports directly to the Homeless Older Adults Manager.

TYPICAL TASKS

Person-Centered, Problem-Solving Conversations Facilitate person-centered, problem-solving conversations to identify participant needs and goals.

Individualized Housing Support Provide individualized, housing-focused support for older adult program participants.

Housing Barriers Assessment and Service Planning: Complete strength-based housing barriers assessments and develop corresponding individualized service and housing plans.

Referral Coordination and Support Facilitate access to mainstream services by coordinating referrals to other providers and supporting participants in following through with referrals.

Progress Monitoring and Evaluation Monitor and evaluate program participants' progress toward achieving their goals.

Data Collection and Reporting: Collect and input required data into the Homeless Management Information System (HMIS) and other relevant platforms.

Community and Stakeholder Engagement Participate in local Continuum of Care planning efforts, forums, and elder coalition meetings.

Community Outreach and Program Evaluation Support community outreach and assist with program evaluation as needed.

Teamwork and Interdepartmental Collaboration Collaborate effectively with internal teams and other departments to ensure holistic support for participants and smooth program operations.

Additional Duties Perform other duties as assigned by the supervisor to support the mission of the program.

KNOWLEDGE, SKILLS, AND ABILITIES

  • A passion for working with underserved populations and a commitment to the Senior Services mission, core principles, and values.
  • Skilled in active listening and interviewing techniques, such as Motivational Interviewing.
  • Strong verbal and written communication skills with the ability to follow written and oral instructions and procedures.
  • Knowledge of HIPAA regulations, ethical standards, and Person-Centered practices.
  • Proficiency in Microsoft Office and web-based software programs; familiarity with HMIS preferred.
  • Excellent organizational and planning skills to manage time effectively and meet deadlines.
  • Adaptability and flexibility to work in a dynamic environment.
  • Ability to work independently and as part of a team.
  • Professional demeanor in interactions with participants, colleagues, supervisors, and external partners.
  • Detail-oriented with the ability to multi-task and prioritize effectively.
  • Strong problem-solving and conflict-resolution skills.
  • Familiarity with community resources and elder-specific services is highly desirable.

WORK ENVIRONMENT

This role is based in our main office/field five days each week to foster team collaboration and firsthand engagement; however, we value flexibility and understand the need for remote work under certain circumstances. Therefore, we offer the option to work remotely when necessary to accommodate the work that is to be accomplished. The organization’s main office is located in Norfolk, VA.

Requirements

EDUCATION AND EXPERIENCE

  • Bachelor’s degree in social work, human services, or a related field. Equivalent combinations of education and experience may be considered.
  • At least one year of experience in community-based services is required.
  • Excellent customer service and telephonic communication skills
  • Must possess a valid driver’s license, auto insurance, a good driving record, and pass an acceptable criminal background check
