Heritage Health Network pairs culturally relevant care teams with smart technology to coordinate medical, behavioral, and social support for Medi‑Cal and Medicare members across California. We prevent avoidable ER visits, reduce disparities, and stop diabetes‑related amputations through person‑centered, community‑based care for the highest‑need populations—youth and adults with avoidable ED use, families involved in child welfare, birthing people facing inequities, adults at risk of long‑term care placement, people experiencing homelessness, and those with or at risk of limb loss.
