Eliot’s Homeless Services Division is seeking a dedicated Housing First Coordinator to join our Community Support Team which provides housing stabilization support to unsheltered adults who are in transition to housing pre- and post- move in. Eliot is committed to a Housing First Model that prioritizes permanent housing as the initial step in assisting individuals experiencing homelessness.

In this role you will provide person-centered, strengths-based case-management. The ultimate goal of our programming is to provide each individual with coaching, education, tools and access to community resources that will help them successfully maintain long-term sustained housing, financial independence and emotional well-being.

Responsibilities:

  • Empower residents with exercising autonomy and independence in all aspects of their life and assists with identifying choices and developing skills in self-advocacy.
  • Conduct initial needs assessment and collaboratively develop a plan with the resident to obtain benefits/entitlements and other community-based resources.
  • Collaborate across the staff team to ensure that resident perspective is always incorporated, that preferences are recognized and that choices are maximized.
  • Support new residents in all phases of transitioning into housing in collaboration with partnering organizations.
  • Assist clients in being fully integrated into their communities by supporting them with resource utilization and/or obtaining memberships and natural supports.
  • Provide practical help and mentoring/modeling, advocacy, coordination, side-by-side individualized support, problem solving and direct assistance to help clients obtain skills needed for daily independent living.
  • Promote concepts of health and wellness by assisting clients with accessing information and services related to medical care including HIV care resources, nutrition, and exercise as appropriate.
  • Intervene in potential crisis situations, utilizing trauma informed, verbal de-escalation techniques.

Qualifications:

  • Bachelor’s degree or Master’s degree in a human services field or social work or related field, preferred
  • For candidates with lived experience with substance use disorder who have 2 years sustained sobriety: High school diploma/GED and at least one year of paid or volunteer experience working with adults with mental illness and/or substance abuse.

Compensation:

  • Bachelors: $48,000.00 annually
  • Masters: $50,000.00 annually

Schedule: Monday-Friday, 9am-5pm
