Job Description

Pay Range: $24.88 - 28.61/hr

Schedule: Monday - Friday; 8:00 am - 4:30 pm

Location: 1607 W Howard St, Chicago IL 60626 or 8541 S State St. Chicago IL 60619

(Please note, the position is available to work at either our Rogers Park or Chatham office listed above)

The Housing Coordinator will coordinate housing efforts for all scattered-site Trilogy housing programs Coordinated Entry System (CES) and Homeless Management Information System (HMIS) related navigation. They will collaborate with different teams/programs, and with representatives from the city, state, and county to secure safe and affordable housing for clients. They will be responsible for receiving and processing CES matches in HMIS and ensuring proper documentation prior to enrollment. They will provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illness and co-occurring substance use utilizing a harm reduction approach. The Housing Coordinator may represent Trilogy at Continuum of Care meetings and other assigned state and local events and committees as needed.

Responsibilities:

  • Manage aspects of Trilogy’s scattered site housing programs, including managing CES referrals and internal referrals, manage participants to successfully maintain housing in accordance with the housing first model of care, database management, etc.
  • Support clients in locating and securing housing in the community.
  • Communicate with landlords and property managers to discuss subsidized housing programs.
  • Meet with clients to provide direct and supportive services related to housing, i.e., lease negotiation process, move-ins, move outs, and ensure new clients are linked to intake for clinical assessments.
  • Prepare and report information in HMIS to ensure the department is meeting benchmarks and standards for grant programs.
  • Knowledge and ability to navigate the Homeless Management Information System (HMIS) and manage all referrals across programs with support of team.
  • Execute all new lease and renewal paperwork in a timely and accurate fashion to ensure lease and grant compliance.
  • Support in setting up client apartments to prepare for move in.
  • Provide effective and collaborative communication with external and internal programs to support a smooth process of record gathering to ensure new clients are document ready when moving into the program.
  • Use knowledge of the Fair Housing Act, Just Housing Ordinance, and RLTO to support teams and clients with tenant advocacy.
  • Provide support and education to the teams regarding housing issues to help secure housing for their clients. May develop and conduct team-based trainings and information sharing sessions.
  • Actively participates in relevant trainings, housing sessions led by internal and/or external partners to build individual and institutional knowledge of housing practices. Perform other related duties and/or projects as assigned.

Qualifications:

  • Must have one of the following requirements:
    • High School Diploma or GED.
    • Associate or bachelor’s degree in any field.
  • Experience working in social services and/or housing administration preferred

  • Experience with housing, case management, social services with people with mental health and/or substance use diagnoses

  • Valid Illinois driver’s license with daily access to a well-maintained vehicle with $100,000/$300,000 liability insurance
  • Exceptional customer service skills and ability to interact with a diverse group of internal/external stakeholders
