Job Description

Pay Range: $24.88 - $28.61/hr

Schedule: Monday - Friday, 8am - 4:30pm

Office Location: 1607 W Howard St. Chicago IL 60607

The Comprehensive Class Member Transition Program is an Illinois State grant funded program designed to support Williams and Colbert Class members residing in Long Term Care Facilities into independent housing with wraparound community supports. The program offers an array of services with the diverting agency such as community mental health supports through an integrated healthcare model and long-term housing supports through transitional programs or subsidy services.

The Housing Coordinator will coordinate housing efforts for the CCMTP team by developing and maintaining relationships with landlords and private management companies who will work with our clients. They will collaborate with different teams/programs, and with representatives from the city, state, and county to secure safe and affordable housing for clients. The Housing Coordinator will also manage all aspects of the Bridge Subsidy Program through Catholic Charities and Statewide Referral Network (SRN).


Responsibilities

  • Develop and maintain relationships with landlords and private management companies throughout the city to secure safe, affordable, and fair housing for clients; including Catholic Charities and other entities.
  • Maintain a caseload and manage all aspects of initial housing placement for CCMTP members; i.e. locate housing units, submission of Catholic Charities paperwork, collaborating with AP for fees, and lead successful lease signings
  • Meet with clients regularly in person to provide direct and supportive services related to housing while clients are still in Long Term Care facilities
  • Communicate and collaborate with the CCMTP care team to enhance client care, service delivery, and facilitate program operations and objectives. Provide new housing opportunities to the team seeking new leads.
  • Provide support and education to the team regarding housing issues to help secure housing for their clients. May develop and conduct team-based trainings and information sharing sessions.
  • Manage the relationship between the tenant, the client, and the team until the client is stably housed.
  • Collaborate with CCMTP Housing team and PSH Housing team to share housing leads and other housing-related resources to benefit the teams and agency
  • Perform timely documentation in CareLogic and Caspio
  • Actively participates in relevant trainings, housing sessions led by internal and/or external partners to build individual and institutional knowledge of housing practices.
  • Perform other related duties and/or projects as assigned

Qualifications

  • Must have one of the following:
    • GED or High School Diploma with 2 years of relevant professional experience
    • Associate’s or Bachelor’s degree in any field
  • Experience working in social services and/or housing administration preferred
  • Experience with housing, case management, social services with people with mental health and/or substance abuse diagnoses
  • Valid Illinois Driver’s License with daily access to a vehicle. Must have $100,000/$300,000 auto liability insurance.
  • Exceptional customer service skills and ability to interact with a diverse group of internal/external stakeholders
