The City of Riverside is accepting applications for the position of HOUSING CASE MANAGER (Confidential)* to fill one (1) vacancy in the Department of Housing and Human Services.The eligibility list established may be used to fill the current and/or upcoming vacancies within this classification for up to six (6) months.

The ideal candidate will have experience providing and conducting housing case management, completing individual service plans, counseling, crisis intervention, resources referral, and case management to formerly homeless individuals and families. This position may include weekend hours to address emergency issues.

Housing Case Manager, under general supervision, to conduct mobile outreach and provide intake and assessment, counseling, crisis intervention, resource referral, case management and assistance with obtaining housing, developing life skills/income, and achieving greater self-determination to homeless individuals and families on the streets, in service venues and other locations where they can be found in the City of Riverside; or to a pre-selected caseload of applicants and/or recipients.

Examples of duties

Typical duties may include, but are not limited to, the following:
  • Review cases and formulate plans for clients to participate in educational and psychosocial assessments and treatments, work skills training, parenting classes, anger management workshops, diversity training, and/or substance abuse treatment.
  • Conduct home visits with participants to provide assistance, answer questions, assess needs, and provide orientation on available services for participant and family members.
  • Serve as an advocate for participants in securing social services, provide encouragement, and support to participants in securing and/or following through on services.
  • Assist participants in their development of short and long term personal goals; provide supportive counseling and assist participants in recognizing and addressing home, family, and social conditions contributing to problems; instruct participants on appropriate methods of social and life skills.
  • Transport participants, as needed.
  • Participate in community activities and projects.
  • Maintain records and write reports on status of clients; prepare correspondence and reports as required; when qualified to do so, serve as an interpreter for non-English speaking family members.
  • Serve as liaison between the participant, the City of Riverside, and other agencies, individuals, organizations, or community groups.
  • Maintain effective working relationships with participants, family members, project staff, and collaborating agencies.
  • Provide training and field assistance to Outreach Workers on an as needed basis.
  • Participate in project presentations, as required.
  • Assist in the preparation of statistical reports and other documents.
  • Provide community education and outreach on the negative impact of gang involvement.
In addition, when assigned to the Housing and Human and Housing Services Department:
  • Work with the City’s Housing Locator to search for rental housing units for rental assistance program participants.
  • Assist client with lease signings to ensure they have a clear understanding of the lease terms.
  • Conduct an individual needs assessment and provide referrals to service providers based on the needs identified.
  • Maintain client case management records in the Homeless Management Information System; prepare correspondence and reports as required; when qualified to do so, serve as an interpreter for non-English speaking family members.
  • Conduct weekly case management case conferencing meetings every week with outreach workers, contracted housing case managers and shelter provider’s case managers.
  • Meet with contracted housing case managers to provide assistance and ensure staff is entering case notes into the partners shared drive.

In addition, when assigned to Supportive Housing Program:
  • Provide case management and supportive services to support stability from substance abuse, physical/mental health and functional concerns resulting from homelessness.
  • Work with individuals and families to achieve short-term and long-term goals, helping them access the necessary services available such as counseling, education, employment and life skills goals.
  • Create referrals and act as liaison with various external agencies including mental health, public health and others to facilitate the client's needs to access community-based resources appropriate to the individual.
  • Work closely with Supportive Housing Program Property Management to ensure clients are stable and abiding by Supportive Housing Program contract and rental lease agreements.
  • Assist in the oversight of Supportive Housing Program Grants.
  • Prepare Annual Progress Reports (APR) for Supportive Housing Program Grants.
  • Work with Homeless Management Information Systems (HMIS).
  • Conduct Vulnerability Index Service Prioritization Decision Assistance Tool (VI-SPDAT) in the Riverside County Coordinated Entry System and update client's information.

Qualifications

Recruitment Guidelines:

Education: Equivalent to an Associate Degree (60 semester units) from an accredited college or university with major work in education, sociology, or a related field.

Experience: Three years of full time experience as an Outreach Worker, Case Manager, or in performing client directed services in either a social services or mental health agency. Two years' experience may substitute for the education requirements on a year for year basis.


Necessary Special Requirements
Possession of an appropriate, valid, Class "C" California Motor Vehicle Operator's License, and reliable transportation.

Must be available to work evenings and weekends, and provide services to project clients at numerous locations, including but not limited to, community parks and recreational centers, School District campuses, and other locations as required.
