Summary

A Housing Case Manager, under general supervision and review, provides and/or arranges comprehensive community and housing-related services for individuals with mental illness, intellectual/developmental disabilities, and/or substance use disorders. Services include assessment, person-centered treatment planning, housing assessment and planning, housing navigation, coordinating, monitoring, review, advocacy, and assistance with obtaining and maintaining safe, stable, and affordable housing. A Housing Case Manager performs the functions of both case management and housing specialization, assisting consumers in identifying housing needs and barriers, locating appropriate housing opportunities, completing housing applications, accessing housing resources and rental assistance programs, communicating with landlords and property managers, and developing strategies to promote long-term housing stability. A Housing Case Manager also provides supportive services, crisis intervention, tenancy support, housing stabilization, eviction prevention, and mental health and housing-related education for consumers, families, landlords, property managers, and significant others, and provides on-call services as assigned.

A Housing Case Manager performs, coordinates, and monitors agency documentation of services provided and performs related work activities as assigned. A Housing Case Manager may also train, mentor, collaborate with, and provide housing-related consultation and support to agency staff and community partners.

Examples of duties

1. Possess a Bachelor degree from an accredited college or university with a major in a human service field such as psychology, social work, education, etc., OR be eligible to graduate within 90 days of application AND able to submit an official transcript upon appointment to position OR possess five (5) years of relevant experience providing supports and/or services to persons with mental illness, developmental disabilities, substance use disorders, individuals experiencing homelessness or housing instability, or other related human services populations.
2. Have one (1) year of experience in providing supports and/or services to persons with mental illness, developmental disabilities, or substance use disorders; please note, internships count as work experience if they are documented in the Work Experience section of the employment application. For candidates qualifying under the five (5) years of relevant experience option identified above, this requirement may be satisfied as part of the five (5) years of qualifying experience.
3. Possess a valid driver's license or the ability to obtain a Michigan Driver License within 30 days of employment.
4. Must have reliable transportation that may have to be used to carry out job duties of this classification.

Preferred but Not Required

1. Bilingual candidates highly desired.
2. Lived experiences with mental illness/developmental disabilities/substance use disorders valued.
3. Individuals in Recovery and individuals with experience in Armed Services valued.
4. Experience providing housing navigation, housing case management, homelessness services, supportive housing, or housing stabilization services preferred.
5. Experience working with landlords, property managers, housing authorities, shelters, or community housing providers preferred.
6. Knowledge or experience with affordable housing, rental assistance, subsidized housing, supportive housing, and homelessness response systems preferred.

Qualifications

PHYSICAL ACTIVITIES

An employee in this class performs generally sedentary work activity requiring occasional lifting of objects weighing thirty-five (35) pounds or less. Duties may also require travel to consumer residences, prospective housing locations, shelters, community agencies, landlord or property management offices, and other community settings.

ENVIRONMENTAL CONDITIONS

A Housing Case Manager works within the Client Services Offices and program locations of HealthWest and in community-based housing settings, although travel throughout the County by use of personal resources is required.

Work may occur in consumers' homes, apartment complexes, shelters, community agencies, housing provider offices, and other community environments while performing housing search, placement, stabilization, and case management activities.
