The Housing Case Manager provides permanent supportive housing program participants—receivingtenant‑basedrentalassistance—with case management and supportive services to helpmaintainhousing stability and prevent the occurrence or recurrence of homelessness. This role ensureshigh‑qualityservice delivery,accuratedocumentation, and strong coordination with internal teams, property owners, and community partners.

Areas of Accountability

Case Management & Participant Support

  • Assess participants’ eligibility and program appropriateness; obtain required enrollment documentation.
  • Assistparticipants in securingappropriate housingbased on household size, composition, geographic preference, and other determinants.
  • Provide case management throughon‑sitevisits, phone consultations, and home visits.
  • Develop individualized service plans with participant input addressing residential stability, income, education, and health-related goals.
  • Demonstrate knowledge of community resources and coordinate linkages to services.

Housing Program Operations

  • Develop andmaintainstrong working relationships with property owners and property management.
  • Assistclients with the housing search and rental application process.
  • Educate clients on Fair Housing laws, tenant rights, and tenant responsibilities.
  • Determinerent subsidy amountsin accordance withfederal guidelines.
  • Perform Housing Quality Standards (HQS) inspections prior tomove‑inand annually.
  • Secure all required documentation (e.g., W‑9, lease agreements, identification documents).
  • Prepare andsubmitmonthly rent and utilityassistancecheck requestsin a timely manner.
  • Maintain complete,accurate, andup‑to‑dateparticipanthard‑copyfiles.
  • Maintain professional objectivity in emotionally charged situations.
  • Perform other duties as assigned.

Data Management & Compliance

  • Maintain all data management requirements within the Homeless Management Information System (HMIS).
  • Enter complete andaccurateclient information in compliance with agency protocols.

Education, Licensure/Certification Requirements

  • Bachelor’s degree; major in social work or related field preferred.
  • At least 1 year of case management or customer service experiencerequired.
  • 1 year of recent housing experience with local, state, and/or federal programs preferred.
  • 1 year of experience working with individuals and/or families experiencing homelessness preferred.
  • Bilingual (English/Spanish) desired
