Copa Health is seeking a dedicated and compassionate Housing Care Navigator to support individuals experiencing homelessness in achieving long-term housing stability. In this role, you’ll provide trauma-informed care, conduct housing assessments, and connect participants with critical community resources. You will play a vital part in empowering participants to overcome barriers, secure housing, and thrive.Schedule: Full-Time Starting Pay: $22-$24 DOEWhat You’ll Do
Conduct intakes and needs assessments for participants.
Create individualized housing plans tailored to each member’s goals.
Provide trauma-informed, harm-reduction support and crisis intervention.
Coordinate with landlords, service providers, and community partners.
Assist participants in securing identification and housing documentation.
Advocate for tenant rights and educate members on lease responsibilities.
Facilitate workshops, peer support groups, and orientation sessions.
Maintain accurate case notes and documentation in the Homeless Information Management System (HMIS).
Meet consistently with members to address barriers and support housing goals.
Transport or accompany participants as needed for housing search and resource access.
Qualifications
B achelor’s degree in Human Services, Social Work, Psychology, or related field OR equivalent combination of education and experience.
At least 1 year of experience working with individuals experiencing homelessness and/or behavioral health needs.
Ability to obtain a Level 1 Fingerprint Clearance Card within 8 weeks of hire
TB test clearance (upon hire and annually).
Valid driver’s license and acceptable driving record.
Must pass pre-employment drug screening.
