Provides medication management services in coordination with other health care professionals to deliver safe, appropriate, and cost-effective medication therapy to patients. Responsibilities may include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Participates in collaborative practice and/or protocol-driven practice as appropriate. Additionally, pharmacists are active members in quality improvement, research activities, and education as described in the job specific competencies.
Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program or Bachelor of Science (B.S.) with a minimum of 2 years relevant experience.
Other advanced degrees (e.g., Master’s, Ph.D.) and applicable Board of Pharmacy Specialties (BPS) or other certification applicable to the position as determined by the supervisor preferred. Completion of pharmacy residency and specialization (e.g., PGY-2 and respective BPS certification), if applicable, preferred. Evidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations preferred. Relevant applicable pharmacy experience working in a progressive and collaborative practice environment preferred. Experience and/or interest in teaching and/or precepting preferred. Commitment to excellence, accuracy, attention to detail and teamwork required. Ability to communicate effectively, work collaboratively with others, organizes time well, practice with an agile mindset, solve problems, and work independently with minimal supervision required.
License/Certification: Current pharmacist license by the Board of Pharmacy in the state(s) of the job's practice site, or eligible to obtain within a time frame compliant with the site’s applicable employee licensure policy. May not independently fulfill responsibilities reserved only to a pharmacist until license is obtained.
