Job Description

Would you like to be part of a growing, locally owned company dedicated to providing compassionate patient care? James River Home Health & Hospice is seeking a Full Time Hospice Team Assistant to join our team!

The Hospice Team Assistant provides administrative and operational support to the hospice interdisciplinary team to promote efficient daily operations, timely communication, accurate documentation, and exceptional service to patients and families.

This position serves as a central resource for the hospice team by assisting with scheduling, communication, documentation tracking, supplies, coordination of services, and other administrative responsibilities. The Team Assistant helps ensure clinical staff remain focused on direct patient care while maintaining organized and consistent office processes.

Schedule: Monday - Friday, 8:00am - 5:00pm

Salary Range: $60,000 - $70,000 per year

Essential Duties and Responsibilities

  • Provides daily administrative support to the hospice clinical team and
  • Ensure office coverage during business
  • Answers phone, greet visitors and generate correspondence as necessary for office
  • Assists with coordinating patient visit schedules and communicates scheduling changes to appropriate team members.
  • Tracks required and missed visits, documentation, orders, certifications, and other outstanding items and communicates deficiencies to the appropriate clinical
  • Able to collaborate with administrator, clinical manager and medical director regarding IDG preparation including scribe responsibilities. Assists with preparation and organization of IDG/IDT meetings, including agendas, reports, attendance, and follow-up
  • Coordinates communication between nurses, hospice aides, social workers, chaplains, physicians, facilities, pharmacies, DME providers, and other members of the care team as
  • Ensure face -to- face encounters are completed timely, implemented and communicated to physicians and NP's.
  • Assists with ordering and tracking patient supplies, DME, and other non-clinical
  • Supports admission and discharge processes by ensuring required administrative documentation is received, organized, and routed appropriately.
  • Monitors incoming calls, messages, faxes, and other communications and ensure they are directed to the appropriate team member promptly.
  • Responsible for ordering and reconciliation process of office supply management with administrator's
  • Assists with maintaining patient records and ensures documents are uploaded, filed, and routed correctly within the EMR.
  • Tracks outstanding physician orders and assists with obtaining signatures and returning completed documents.
  • Assists leadership with reports, census information, staffing schedules, meeting preparation, and other operational
  • Supports preparation for weekends and holidays by helping verify schedules, supplies, medications, DME, and other identified patient needs have been addressed by the appropriate team members.
  • Maintains confidentiality and complies with HIPAA, organizational policies, and applicable hospice regulations.
  • Provides professional and compassionate customer service to patients, families, facilities, referral partners, and staff.
  • Identifies operational concerns and promptly escalates them to the appropriate clinical
  • Maintains office
  • Proven consistency of positive (or upward trending) client satisfaction
  • Performs other administrative duties as

Role Boundaries

The Hospice Team Assistant is a non-clinical support position. The Team Assistant does not independently assess patients, determine hospice eligibility, provide clinical advice, change plans of care, interpret physician orders, or make clinical decisions. Clinical concerns must be immediately referred to the appropriate licensed clinician.

Qualifications

  • High school diploma or equivalent required; additional healthcare or administrative education preferred.
  • Previous hospice, home health, healthcare, medical office, or administrative experience
  • Knowledge of hospice operations and terminology
  • Experience with electronic medical record systems Home Care Home Base
  • Strong organizational, communication, and customer service
  • Reliability, problem solving and decision-making
  • Ability to manage multiple priorities in a fast-paced
  • Strong attention to detail and ability to follow established
  • Proficiency with Microsoft Office and general computer
  • Ability to work collaboratively with an interdisciplinary
  • Compliance with accepted professional standards and
  • Satisfactory references from employers and/or professional
  • Satisfactory criminal background
