Hospice RN Case Manager
Dacula / Duluth / Buford, Georgia

We’re hiring a Hospice RN Case Manager in Dacula, Duluth, and Buford, Georgia who wants meaningful patient relationships, autonomy in the field, and a schedule that supports life outside of work.

You’ll step into a clinical role focused on providing hospice care in patients’ homes throughout northern Gwinnett County. You’ll have the support of an experienced interdisciplinary team while maintaining independence in managing your caseload.

• You are a Registered Nurse licensed in Georgia
• Hospice experience is preferred but not required
• Home Health, ICU, CCU, Oncology, and Med Surg experience are welcomed

Compensation & Benefits

• Base salary: $90,000–$95,000
