Hospice RN Case Manager
Stone Mountain / Clarkston / Tucker / Chamblee / Lilburn, Georgia

We’re hiring a Hospice RN Case Manager who wants autonomy, meaningful patient relationships, and the opportunity to provide compassionate end-of-life care.

You’ll step into a clinical role focused on providing hospice care in patients’ homes throughout Stone Mountain, Clarkston, Tucker, Chamblee, Lilburn, and the surrounding communities. You’ll collaborate closely with an experienced interdisciplinary team while maintaining independence in managing your caseload.

What makes you a great fit

• You are a Registered Nurse with an active Georgia license
• Hospice experience is preferred but not required
• Home Health, ICU, CCU, Oncology, and Med Surg backgrounds are welcomed
• You have strong communication and assessment skills
• You are a compassionate patient advocate
• You enjoy working independently while collaborating with a multidisciplinary team

Compensation & Benefits

• Base salary: $88,000–$93,000
