Hospice RN Case Manager
Sandy Springs / Dunwoody / Peachtree Corners / Norcross / Doraville, Georgia

We’re hiring a Hospice RN Case Manager who wants flexibility, meaningful patient care, and long-term stability.

You’ll step into a clinical role focused on providing hospice care in patients’ homes throughout Sandy Springs, Dunwoody, Peachtree Corners, Norcross, Doraville, and the surrounding communities. You’ll partner with an experienced interdisciplinary care team while maintaining autonomy in managing your caseload.

What makes you a great fit

• You are a Registered Nurse with an active Georgia license
• Hospice experience is preferred but not required
• Home Health, ICU, CCU, Oncology, and Med Surg experience are welcomed
• You have excellent communication skills
• You bring a compassionate, patient-centered approach to care
• You enjoy working independently while collaborating with a multidisciplinary team

Compensation & Benefits

• Base salary: $88,000–$93,000
