Location: San Diego

Schedule: Monday - Friday or Tuesday - Saturday with On-Call Rotation

What You Will Do

• Provide compassionate, patient-centered nursing care to hospice patients and their families in the home and community setting.
• Complete comprehensive and ongoing assessments to identify patient and family needs.
• Develop, implement, and regularly update individualized hospice and palliative care plans.
• Assess and manage pain, symptoms, medications, and treatments in accordance with the patient’s plan of care.
• Serve as a Case Manager when assigned and coordinate care across the patient’s interdisciplinary hospice team.
• Educate patients and families on medications, symptom management, disease progression, and comfort-focused care.
• Communicate changes in patient condition and care needs with physicians and members of the interdisciplinary team.
• Maintain accurate, timely clinical documentation and ensure continuity of care.
• Support patients and families with their physical, emotional, and end-of-life care needs.
• Coordinate necessary equipment, supplies, and services to support quality patient care.
• Participate in interdisciplinary team meetings, quality improvement activities, and continuing education.

Required Qualifications

• Is a graduate of an accredited school of professional nursing, BSN preferred.
• Currently licensed to practice as a RN in the state of practice.
• If IV or ET certified, is currently certified and recognized by the state(s) where currently practicing as an RN.
• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
• Possess and maintains a valid driver’s license.

Range: $53-$57 per hour, commensurate with experience.
