As we continue to grow, we are looking for a compassionate, dependable, and experienced Hospice Registered Nurse (RN) to join our team and make a meaningful difference in the lives of our patients and their families.

The Role

As a Hospice Registered Nurse, you will provide skilled, compassionate nursing care to patients with life-limiting illnesses in the comfort of their homes. You will assess patients' needs, manage symptoms, coordinate care, provide education and emotional support to patients and families, and help ensure comfort and dignity throughout the end-of-life journey.

You will work closely with our Hospice Clinical Director and interdisciplinary team, including physicians, nurses, social workers, chaplains, hospice aides, and our administrative staff, to provide coordinated and individualized patient care.

Job Type: Full-Time

What You Will Do

  • Provide skilled nursing care in accordance with each patient's individualized hospice plan of care.
  • Perform comprehensive nursing assessments and continuously evaluate patients' physical, emotional, and comfort needs.
  • Provide symptom management and pain management interventions to promote patient comfort and quality of life.
  • Administer medications and treatments and evaluate patient response and effectiveness.
  • Educate and support patients, families, and caregivers regarding medications, symptom management, disease progression, and end-of-life care.
  • Communicate changes in patient condition promptly to the Hospice Clinical Director and appropriate members of the interdisciplinary team.
  • Coordinate care with physicians, hospice aides, social workers, chaplains, facilities, pharmacies, and other members of the care team.
  • Participate in interdisciplinary team collaboration and care planning.
  • Complete accurate and timely clinical documentation using Kinnser/WellSky on company-provided devices.
  • Maintain compliance with hospice regulations, company policies, clinical standards, and safety requirements.
  • Provide compassionate support to patients and families during one of the most important and sensitive stages of life.

Qualifications

  • Current and unrestricted Registered Nurse (RN) license in the State of California
  • Minimum of 1 year of nursing experience
  • Hospice or Home Health experience is highly preferred
  • Strong clinical assessment, communication, and organizational skills.
  • Ability to work independently in the field while collaborating effectively with an interdisciplinary team.
  • Compassionate and patient-centered approach to end-of-life care.
  • Valid California driver's license, reliable transportation, and ability to travel throughout the assigned service area.
  • Comfortable using electronic medical records and mobile technology for clinical documentation.
