Ohio’s Hospice is looking for compassionate and dependable Personal Care Specialists (STNA/CNA) to join our team. In this role, you’ll provide hands-on care and support to hospice patients in their homes and other care settings throughout Newark and surrounding communities.

What You’ll Do
- Review each patient’s individualized Plan of Care before every visit to ensure appropriate care is provided.
- Provide or assist with personal care, including bathing, dressing, oral hygiene, hair care, meal preparation, feeding, transfers, and exercises.
- Help maintain a clean, comfortable, and safe patient environment, including changing bed linens, adjusting room temperature and lighting, and performing light housekeeping as needed.
- Obtain vital signs and other measurements as ordered and report results accurately and promptly to the supervising RN.
- Observe patients closely and promptly report changes in physical or mental condition to the RN.
- Communicate patient updates and concerns with the RN and other members of the interdisciplinary care team.
- Document patient care, observations, interventions, and communication accurately in the electronic medical record at the point of care.
- Follow each patient’s individualized Plan of Care and maintain accurate documentation.
- Participate as an active member of the interdisciplinary hospice team.
- Support Continuous Quality Improvement activities that promote high-quality hospice services.
- Assist other departments and teams as needed to support comprehensive patient care.
- Help orient and precept new staff members as assigned.
What We’re Looking For
Required:
- High school diploma or GED.
- Valid Ohio driver’s license with a safe driving record.
- Reliable transportation and proof of auto insurance.
- Ability to pass a criminal background check and 10-panel drug screening.
- Basic computer skills and the ability to accurately document patient care electronically.
- Compassionate, dependable, and respectful approach to working with patients and families.
Preferred:
- Certified Nurse Aide (CNA/STNA) certification. Training may be provided for qualified candidates.
- Certified Hospice and Palliative Nurse Aide (CHPNA) certification.
