Establish and maintain relationships with referral sources in the community.
ESSENTIAL DUTIES AND RESPONSIBILITIES
include the following. Other duties may be assigned as necessary.
· Organize, track, and build a complete medical record for appropriate patient transition to hospice; and confirm start of care for each patient referred.
· Obtain complete/accurate demographic information, medical history and primary care physician information to provide to Intake.
· Welcome call; introduce Residential Hospice to the patient/caregiver, explaining scope of our services, and coordinating start of care visit.
· Travel to facilities to obtain signatures for late orders when needed.
· Provide coverage for Hospice Consultants during PTO.
· All other duties as assigned by supervisor.
· Schedule activities and or training in facilities.

·Bachelors degree preferred.
·One plus years of customer service or sales experience.
·Proven ability to interact with individuals at all levels of the organization
·Strong interpersonal skills.
·Maintain a valid driver's license, maintain automobile insurance coverage and have access to a reliable automobile.
·Ability to handle and maintain confidentiality and have strong attention to detail.
·Ability to work in a fast-paced environment with demonstrated ability to juggle multiple competing tasks and demands.
CERTIFICATES, LICENSES, REGISTRATIONS
Must have and maintain in good standing professional license, certificate, or registration, as applicable.

Employee will be required to drive daily for meetings and/or visits to residential homes. Employee will work in varied residential environments based on assigned case load. While performing the duties of this position, the employee may travel by automobile and be exposed to changing weather conditions.
