Overview
The remote Triage Registered Nurse who provides after-hours clinical triage and coordination for hospice patients receiving care in home or facility settings.
To be considered, candidates must:
• Live in Michigan, Indiana, or Pennsylvania
• Hold an active RN license in one of these states
• Preferably hold a compact (multistate) RN license to support coverage across state lines
Licensure note:
Indiana and Pennsylvania participate in the Nurse Licensure Compact. Michigan does not currently participate, so candidates living in Michigan must hold an active Michigan RN license.
Schedule & Coverage
• Rotating 7 days on / 7 days off schedule
• Coverage hours are 4:30 PM to 8:30 AM
• Assigned coverage area determined by branch needs and patient census

Position Responsibilities
• Triage patients requiring urgent and emergent medical advice evenings, weekends, and holidays via telephone
• Process incoming hospice referrals
• Assign and reassign admission, evaluation, subsequent, and discharge visits within the EMR
• Run scheduling reports
• Assist field staff with ordering durable medical equipment (DME) and patient supplies as needed
• Assist with the start of care admission process, displaying proficiency in identifying hospice eligibility requirements set forth by Medicare guidelines
• Educate the patient and family on medication management, treatment protocols, and end-of-life processes, while identifying caregiver's ability to provide safe care
• Engage in effective interdisciplinary communication with various professions of the hospice care team to promote effective care for the patient and family
• Document all health assessments, physician orders, and communication with the patient, family, and medical professionals in a timely manner
• Maintain patient confidentiality, including all HIPAA regulations
• Comply with agency policies and procedures regarding operations/processes, including patient care, patient complaints, incidents, safety, and emergency management
• Manage territory with minimal supervision based on forecasted needs, priorities, and business strategy
QualificationsPROFESSIONAL EXPERIENCE/QUALIFICATIONS:
- 1+ year of previous Hospice nursing experience.
- Minimum of one year of experience in medical/surgical and one year of experience in critical or emergency care with specialized triage training.
- Strong written and verbal communication skills.
- Excellent customer service skills.

Regulatory Requirements
- Graduate of an accredited school of nursing, with a bachelor's degree preferred.
- Current state license as a Registered Nurse
- Must pass a criminal background check & MVR check.
- Completed health statement acknowledging the ability to perform the duties of the position.
- Valid state driver's license.
- Proof of current automobile insurance.
- TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).
