Mt Carmel Hospice -Columbus, OH,

$10,000 sign on bonus

On Call RN staffing support (7 on and 7 off) to provide dedicated nursing support coverage from 5:00 PM to 8:00 AM, seven days per week.

The Hospice On-Call Registered Nurse (RN) provides skilled end-of-life nursing care to hospice patients throughout the Columbus, Ohio service area. Utilizing clinical judgment and critical thinking skills, the RN assesses patient and family needs, develops and implements individualized plans of care, manages symptom control, and coordinates services with the interdisciplinary hospice team. The On-Call RN responds to urgent patient concerns, performs visits as needed, provides caregiver education and support, and ensures timely documentation while promoting comfort, dignity, and quality of life for patients facing life-limiting illnesses.

Key Responsibilities

  • Perform comprehensive patient assessments and evaluate changes in condition.
  • Provide symptom management and end-of-life nursing care.
  • Respond to after-hours, weekend, and holiday patient needs.
  • Coordinate care with physicians, hospice leadership, facilities, and interdisciplinary team members.
  • Complete admissions, routine visits, crisis visits, General Inpatient (GIP), and Continuous Care (CC) visits as assigned.
  • Educate patients and caregivers regarding disease progression, medications, and comfort measures.
  • Ensure accurate and timely EMR documentation and compliance with hospice regulations.
  • Participate in quality improvement initiatives, team meetings, and ongoing professional development.

Qualifications

  • Active Ohio RN license.
  • Current CPR certification.
  • Hospice, home health, oncology, critical care, ER, or med-surg nursing experience preferred.
  • Strong assessment, communication, and autonomous decision-making skills.
  • Ability to travel throughout the greater Columbus market and participate in on-call rotations.
