Want to join our Team? We are currently looking for a caring and compassionate Nurse Practitioner to work with our hospice care division. The Hospice NP provides highly professional telephonic, telehealth, and direct patient care to patients during their hospice admission wherever the patient calls home. The Hospice NP manages symptoms through behavioral and medical interventions and engages in goals of care conversations to direct treatment plans for our complex and seriously ill patient population. This position will primarily cover Calvert and Charles Counties with some coverage in our Inpatient Care Center in Charles Counties.

Office Location: Prince Frederick and/or Waldorf, MD

Status: Full-time

Schedule: 40 hour work week

Salary: Competitive, commensurate with experience, education and credentialing

Main Responsibilities:

  • Respond efficiently to clinical calls from IDT team members, assess patient needs, and prescribe/refill medications as needed. Monitor and fulfill requests from multiple messaging channels.
  • Assess symptom burden and craft treatment plans to reduce unnecessary suffering.
  • Create treatment plans for inpatient care. Round in the inpatient care centers as requested, create high-quality documentation, lead family meetings, and provide bedside patient and family support. Review goals of care and related documentation. Create new advance directives and MOLST forms as needed.
  • Provide coaching in behavioral change or mind-body techniques to reduce illness exacerbations.
  • Authorize prescriptions through the electronic systems.
  • Perform face to face visits and symptom or crisis related visits in person, and complete documentation and coding in EMR within 24 hours. Utilize agency approved formatting for documentation.
  • Complete death certificates within 24 hours of their assignment and expedite their completion in unique circumstances.
  • Manage time proactively to ensure all aspects of the role are completed in a timely way.
  • Participate in advance practice nurse and physician staff meetings.
  • Participate in quality improvement activities and attend meetings as requested.
  • Create and provide education for internal and external audiences.
  • Attend hospice IDT meetings to share clinical findings as requested.
  • Other duties as assigned.

Qualifications

QUALIFICATIONS

  • Active State of Maryland Nurse Practitioner License in good standing.
  • Active Controlled Substance and DEA license in good standing required.
  • Minimum of two (2) years’ experience as an independent nurse practitioner in hospice, palliative, geriatrics, oncology, critical care, or another related field. Inpatient palliative or hospice experience preferred.
  • Board certification in Hospice and Supportive Care required within 2 years of employment.
  • Clean or nearly clean medical malpractice history.
  • Proficient EMR and Microsoft Office technology.
  • Eligible for provider enrollment in Medicare and Medicaid.
  • Demonstrated understanding of and a commitment to the philosophy of Hospice and Supportive Care, Familiarity with hospice, home care and adult social services.
  • Fluent in the ethical and legal issues surrounding end of life care and prescribing controlled substances.
  • Excellent interpersonal, organizational, written and verbal communication skills required.

We are unique in that we provide grief support services to anyone in the community seeking support including those who have experienced a traumatic or sudden loss. The scope of services far exceeds that which is provided for in the Medicare Hospice Benefit.
