Job Description

Requirements

Qualifications:

  • Current New York State Registered Nurse License/Permit.
  • Current valid NYS Driver’s License preferred.
  • Must have own car to conduct site assessments.
  • Minimum of three years relevant clinical experience.
  • Working knowledge of inpatient/out-patient/home care/hospice; application of admission/continued stay criteria and third party payers.

Education:

  • Bachelor’s degree preferred.

Responsibilities:

The Hospice Nurse Liaison are responsible for coordinating applications through the admission review process. Coordinates and maintains referrer relationships with assigned institutions/agencies. Participates in development and marketing Calvary Hospital’s Home Care and Hospice Services. Provides pre-admission assistance to patients, families and referrers.

  • Full-Time, Day Shift

Salary Range:

  • $109,208 - $135,708
