Medical Social Worker (MSW / BSW)
Emotional support. Resource guidance. Advocacy.
Support patients and families through compassionate guidance, emotional support, and meaningful connection during the hospice journey.
The Medical Social Worker provides psychosocial support to hospice patients and families receiving care in home or facility settings, helping address emotional, social, and practical needs throughout the course of care.
Schedule, Hours & Territory
• Standard operating hours are Monday through Friday, 8:30 AM to 4:30 PM
• Territory: Lansing, MI with occasional support in the Grand Rapids market
• Participation in an on-call rotation may be required with additional compensation provided

Key Responsibilities
• Conduct psychosocial assessments
• Provide counseling and emotional support to patients and families
• Assist with advance care planning and coordination of community resources
• Participate in interdisciplinary team meetings
• Document services accurately and timely in the EMR HCHB (Homecare Homebase)
