As a Hospice Social Worker, you will provide a wide range of social work services to patients and families, including emotional, spiritual, and psychosocial support. You will play an integral role on the Hospice IDG (Interdisciplinary Group) by:
Performing initial assessments and assisting in care plan development
Conducting ongoing reassessments and providing counseling as needed
Offering short-term crisis intervention and family counseling
Supporting patients and families through issues of death and dying
Serving as an advocate for the patient/family unit
Participating in bereavement services and community outreach
Coordinating with community agencies and referral sources
Documenting visits, assessments, and interventions clearly and on time
Attend staff meetings, IDG conferences, and trainings as assigned
Participate in orientation and ongoing professional development
Adhere to Hospice policies and maintain professional standards
Exhibit the Hospice philosophy in all interactions and care delivery
Complete clinical notes for all services provided within 48 hours
EDUCATION:
Requirements
• Master's degree in social work (MSW) from an accredited program
• Active Pennsylvania master's-level social work license (LMSW or LCSW)
• Valid driver's license and reliable transportation
• Ability to pass a background check & MVR check; TB Testing per agency policy
• Hospice or healthcare social work experience preferred
• Strong communication, counseling, and crisis-intervention skills
• Ability to work independently and within an interdisciplinary team
• Compassion and professionalism when supporting patients and families at end-of-life
