Job Description

Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.

Title: Hospice Medical Records Coordinator
Schedule: Monday-Friday, 8am to 5pm
Pay: $19.14 to $23.10 per hour
Location: Louisville, CO

The Medical Records Coordinator keeps patient records accurate, organized, secure, and up to date. This role manages both electronic and paper records and helps ensure documentation meets agency, HIPAA, state, federal, and accreditation requirements for hospice and palliative care.

EDUCATION AND QUALIFICATIONS
• 3+ years of experience in medical records, highly preferred
• 1+ years of experience in a medical office or medical records setting, required
• Familiarity with Microsoft Word, Excel, and Windows
• Knowledge of medical terminology
• Comfortable using computers and learning new systems
• Able to pass required background checks and drug screening
• Able to meet agency health screening and immunization requirements
• High school diploma or equivalent

ESSENTIAL DUTIES
• Maintain accurate and complete patient records, both electronic and paper
• Review new patient charts for completeness and accuracy
• Scan, upload, file, and organize clinical documentation in the electronic health record
• Track documents requiring physician signatures and follow up as needed
• Process medical record requests in accordance with HIPAA
• Coordinate face-to-face scheduling and track completion with physicians, NPs, and IDG teams
• Monitor electronic reports and records for accuracy and completeness
• Help ensure charts meet regulatory and billing requirements
• Assist with audits and survey preparation by gathering and verifying documentation
• Answer incoming calls and route them to the appropriate staff
• Perform routine clerical tasks such as data entry, faxing, copying, and filing
• Work closely with clinical and volunteer teams to support timely and accurate record keepin
