Summary
The licensed practical nurse (LPN) is responsible for participating in and providing nursing services as set forth by the North Carolina Board of Nursing/South Carolina Board of Nursing to individuals and their families in a homecare or a facility setting.  Assessment consists of participating in the determination of nursing needs; and planning includes participation in the identification of the patient’s needs related to the findings of the nursing assessment. Implementation of nursing activities consists of delivering nursing care according to an established Plan of Care by the registered nurse overseeing the care of the patient. As delegated by the registered nurse, the LPN participates in the implementation of the Plan of Care by communicating and working cooperatively with members of the interdisciplinary Group (IDG).
Essential Functions
- Delivery of Patient Care
- - Participates in the assessment of the patient’s physical and mental health, including the patient’s reaction to illness and treatment regimens.
- - Records and reports the results of the nursing assessment.
- - Participates in implementing the Plan of Care developed by the registered nurse and/or prescribed by any person authorized by State law, by performing tasks assigned or delegated by and performed under the supervision or under orders or directions of a registered nurse, licensed physician or any other person authorized by State law to provide supervision.
- - Facilitates patient and caregiver independence to the extent possible.
- - Participates in the teaching and counseling of patients as assigned by a registered nurse, physician or other qualified professional authorized by State law.
- - Encourages and allows time for questions when meeting with the patient and family.
- - Reports and records the nursing care rendered, and the patient’s response to that care.
- - Communicates effectively with members of the IDG, patients, caregivers, and other health professionals to ensure continuity of care.
- - Maintains open lines of communication with supervisor. Keeps supervisor apprised of any complex patient/caregiver issues.
- - Applies principles of infection control. Follows guidelines for universal precautions in accordance with accepted professional practices.
- Interdisciplinary Group
- - Attends and participates in the interdisciplinary group conferences [IDG] and special care conferences.
- - Seeking and utilizing the appropriate resources for the referral process, as directed by the RN
- - Participates in identifying specific patient/caregiver needs and making referrals to appropriate team members.
- Documentation
- - Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards.
- Professional and Personal Accountability
- - Demonstrates competency in knowledge and skills within scope of practice.
- - Ensures that VIA Health Partners professional reputation is maintained and projected.
- - Assumes responsibility for professional development and staying abreast of current trends in the healthcare field. Incorporates new information and methods into practice.
- - Maintains current professional licensure, CPR certification and CEU requirements.
- - Exhibits time management skills. Able to plan workday while remaining able to respond appropriately to unforeseen obstacles.
- May perform other duties as required.

QualificationsMinimum

Qualifications
- A Diploma from a Board approved nursing program designed to prepare a person for practical nurse licensure is required;
- A current, active, and unrestricted compact (multistate) license as a Licensed Practical Nurse (LPN) is required.
- The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state’s practice laws and discipline.
- Current CPR required; CHPN preferred.
- Minimum 1-year related experience for LDHH and 2 years for other nursing positions.  Experience in direct care of patients with life limiting illnesses required.  Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.
- Computer proficiency is required.
