Job Description

Join a Hospice Team That Puts Compassion—and People—at the Center

At Day City Hospice, we were founded with a clear mission: create a truly patient-centered hospice experience that maximizes bedside care and stays deeply rooted in the communities we serve. Our Ohio-based leadership team understands the importance of being local—because when patients and families are facing one of life’s most vulnerable moments, they deserve care that is timely, personal, and grounded in community.

We also believe that exceptional care starts with supporting the caregivers. That’s why we provide our team with the right tools, realistic caseloads, and a culture built on respect and collaboration. No overwhelming quotas. No sacrificing quality for volume. Just meaningful work with the time and support to do it well.

If you’re looking for a place where your work truly matters, you’ll feel at home here.

A Flexible, Low-Commitment Schedule

Just 3 twelve-hour shifts per month

This unique schedule is ideal for nurses looking to supplement their income, maintain clinical skills, or enjoy meaningful patient care without the demands of a full-time role. It’s a great fit for those seeking flexibility, work–life balance, or a lighter workload while still making a real impact.

What We Offer

• Supportive, team-focused culture

• 401(k) with generous company match

• Mileage reimbursement

• Employee referral program

• Hospice certification benefit with salary increase

• Merit-based pay increases

• Night shift differential

• Employee discount programs

What You’ll Do

• Provide direct patient care in alignment with the State Nurse Practice Act

• Deliver skilled nursing care based on comprehensive assessments and individualized plans of care

• Document patient services accurately and promptly

• Educate patients and families on diagnoses, care plans, and symptom management

• Collaborate with the interdisciplinary team to ensure coordinated, holistic care

• Provide effective pain and symptom management and evaluate patient responses

What You Bring

• Graduate of an accredited practical or vocational nursing program

• Three years of nursing experience (hospice, community health, or med-surg preferred)

• Current LPN/LVN license in the state of practice

• Current CPR certification

• Strong communication and organizational skills

• Valid driver’s license and insured, reliable vehicle

• Understanding of the hospice philosophy and the needs of the terminally ill

If you’re seeking a meaningful nursing role with flexibility, purpose, and a supportive team behind you, Day City Hospice would love to welcome you.
