Overview
Come join our growing team! The Care Team Hospice is seeking a full-time Licensed Practical Nurse (LPN). The Licensed Practical Nurse (LPN) provides skilled nursing support to hospice patients receiving care in their homes or facility settings under the direction of a Registered Nurse.
Schedule & Coverage
• Standard operating hours are Monday through Friday, 8:30 AM to 4:30 PM
• Sanford, Auburn, and Saginaw, MI

Position Summary
This role supports patient care delivery through skilled nursing interventions, observation, and documentation.
Key Responsibilities
• Provide nursing care as delegated by the Registered Nurse (RN)
• Monitor patient status and report changes
• Support comfort and symptom management
• Document care provided in the EMR
• Collaborate with the interdisciplinary team
QualificationsRegulatory Requirements
- Licensed or registered in the State of Michigan.
- Must pass a criminal background check & MVR check.
- Completed health statements acknowledging the ability to perform the duties of the position.
- Valid state driver's license.
- Proof of current automobile insurance.
Licensed Practical Nurse (LPN) Hospice Expertise
- Must be computer literate and able to maintain simple records in English.
- Promotes patient/family education using various verbal and written communication techniques that take into account patients' and families' cultural, ethnic, and/or personal needs or preferences.
- Collaborates with the patient/family to learn appropriate self-care techniques.
- Submits documentation on time per agency policy.
- Knowledgeable of current federal, state, and agency regulations concerning patient care, including incident management and reporting.
- Maintains regulatory requirements. Including complying with Legal Authority: Act 519, Public Acts of 1982; MCL 400.11, which states: All persons employed, licensed, registered, or certified to provide health care must report a suspicion or reasonable cause to believe that physical, mental, or sexual abuse, neglect, or exploitation of an adult has occurred.
- Participates in agency's other mandated activities, i.e., in-services, Quality Assurance, and Performance Improvement (QAPI); staff meetings.
EDUCATION:
- Graduate of an accredited school of practical nursing.
- Must hold a license to practice practical nursing in Michigan by LARA.
- Should have a minimum of one (1) year of work experience as an LPN, preferably in a Hospice setting. Hospice experience preferred
- Knowledge of general nursing practice.
