The Hospice Liaison Nurse serves as a vital connection between patients, families, healthcare providers, and hospice services. As a trusted clinical resource, you will conduct clinical assessments, facilitate hospice admissions, and coordinate discharge planning to ensure seamless transitions from acute care settings to hospice care.

The primary responsibility of this role is referral processing and admission scheduling, ensuring timely and efficient access to hospice services for eligible patients. Working collaboratively with physicians, case managers, social workers, healthcare partners, patients, and their loved ones, you will provide education, guidance, and advocacy that support patient-centered care and positive end-of-life outcomes.

Key Responsibilities

  • Process incoming referrals and coordinate hospice admission scheduling.
  • Conduct comprehensive clinical assessments to determine hospice eligibility and patient needs.
  • Facilitate hospice admissions and ensure smooth transitions from hospitals and other healthcare settings.
  • Collaborate with physicians, case managers, discharge planners, and interdisciplinary healthcare teams to coordinate care.
  • Educate patients, families, and referral sources on hospice services, benefits, and eligibility criteria.
  • Support discharge planning efforts and address barriers to timely hospice placement.
  • Build and maintain effective relationships with hospital staff, healthcare providers, and community partners.
  • Ensure compliance with organizational policies, regulatory requirements, and clinical standards of practice.
  • Advocate for patients and families by promoting dignity, comfort, and quality of life.

Work Environment

This position is primarily hospital-based, serving as an on-site liaison between healthcare providers and hospice services. The role will be based at Huntersville Medical Center; however, the team member may be required to work at other locations as needed to support business operations. Occasional field-based admissions and patient visits may also be required based on patient and organizational needs.


What You'll Do

  • Education: H4 Year / Bachelors Degree, preferred. Graduate of an accredited school of nursing required; BSN preferred.
  • Experience: Minimum three years' experience in home care or hospice, required. Previous experience in managed care, infusion care, and case management and previous home health/hospice case management experience, preferred.
  • Licensure/Certification: Current RN licensure in appropriate state, required.
  • Additional Skills (required):
    • Ability to provide direct patient care, demonstrate excellent nursing skills
    • Excellent verbal and written communication skills, problem solving skills, and basic math skills.
    • Understands hospice philosophy and issues of death/dying and demonstrates competency in pain and symptom management with EOL care.
    • Ability to interpret information to identify each patient’s requirements for care relative to his/her age specific needs.
    • Ability to prioritize, organize and handle varying and unpredictable situation such as emergencies or crisis situations.
    • Must be flexible, versatile, adaptable to change, with good critical thinking, problem solving and diplomacy skills.
    • Knowledge of procedures and nursing standards of practice, both general and specific to the specialties of medical- surgical nursing.
    • Ability to interact effectively with diverse individuals at all levels of authority, in teams and on committees.
    • Actively identifies new areas for learning; regularly creating and taking advantage of learning opportunities; using newly gained knowledge and skill on the job and learning through their application.
    • Must have computer experience and have reliable transportation to and from all work related assignments and activities.
    • Must be flexible to travel as needed between different locations, as assigned, in all weather conditions and on roads in varying degrees of repair.
    • Must be available for after hours call on rotational basis.
    • Specific vision abilities required by this job include close vision, distance vision, night driving, and ability to adjust and focus.
