The Hospice Liaison will be responsible for developing referral relationships and driving census growth throughout the Jacksonville market. This individual will work closely with skilled nursing facilities, assisted living communities, hospitals, physicians, case managers, and other referral sources to increase awareness, generate admissions, and expand market share.
This role is ideal for a self-motivated sales professional who enjoys building territory, creating new partnerships, and helping grow a developing market with significant upside potential.
Key Responsibilities
• Develop and execute a strategic territory growth plan throughout Jacksonville and surrounding communities
• Build and maintain relationships with skilled nursing facilities, assisted living communities, hospitals, physician practices, and other referral partners
• Generate qualified hospice referrals and admissions
• Conduct regular sales calls and community outreach activities
• Educate referral sources on hospice services, palliative care services, and specialty programs
• Identify new business opportunities and market expansion initiatives
• Collaborate with clinical and operational teams to ensure exceptional customer service and patient experience
• Maintain accurate documentation of sales activities and referral pipeline
• Represent the organization at community events, networking functions, and industry meetings
• Consistently meet and exceed census and admission goals
Qualifications
• Minimum of 2 years of healthcare sales experience required
• Hospice, home health, post-acute care, physician services, wound care, or senior living sales experience strongly preferred
• Established referral relationships throughout the Jacksonville market preferred
• Demonstrated success achieving and exceeding sales goals
• Strong relationship-building and business development skills
• Excellent communication and presentation abilities
• Comfortable with a highly field-based sales environment and active prospecting
• Self-starter with a high level of accountability and initiative
• Ability to work independently while collaborating effectively with a larger team
• Valid driver's license and reliable transportation required
