Job Description

Job Location: RFICC - Pasadena, MD 21122
Position Type: Part Time
Salary Range: $20.00 - $30.00 Hourly

This position is two 12-hour shifts per week from 7pm - 7:30am in our Pasadena Inpatient Care Center.
DUTIES AND RESPONSIBILITIES
- Under the direction of an RN, performs all required interventions and/or procedures to provide for total patient care in the clinical setting according to the plan of care, including turning, transporting, catheter care, bathing and skin care. Provides care at time of death as assigned. Contacts RN Case Manager or designee for any changes that relates to the plan of care.
- Report changes in the patient medical, emotional, spiritual or psychosocial status to the RN Case Manager or designee.
- Documents appropriately and according to the plan of care using technologies provided by HOC which may include phone applications such as Mobile Lite. Documents at the time of visit and reports any barriers to timely documentation immediately to supervisor or designee. Completes paper documentation when applicable according to the documentation guidelines for CNA.
- Follows guidelines to report technology failures per HOC policy such as contacting the HELP desk if unable to perform duties as assigned; such as document on phone or complete In-services or retrieve emails.
- Assists in the collection of data regarding family needs for bereavement services following the patient’s death and conveys information to primary.
- Identifies and recommends medical equipment and supplies that will assist in maintaining patient’s comfort and caregiver’s ability to provide care and communicates this information to the RN.
- Demonstrates knowledge of and always follows Standard Precautions and infection control procedures.
- Uses proper body mechanics when caring for patient. Brings any safety issues to attention of RN Case Manager and Clinical Manager.
- Demonstrates knowledge of home safety issues and follows safety practices for storage, handling and access to supplies appropriate to the situation.
- Consistently utilizes effective time management and performs functions efficiently.
- Reports patient/family/employee incidents as required by HOC policy.
- Records time and mileage appropriately and timely according to HOC policy.
- As per the plan of care, provides patients with clean linen and makes or straightens bed. Demonstrates knowledge of correct bed making procedures (occupied, unoccupied) as outlined in the procedure manual. Maintains clean, neat patient care area.
- As per the plan of care, assists with patient meals. Moves patient in proper position to eat. Sets up tray for patient. Assists or feeds patient as ordered
- Provides for patient privacy at all times.
- Completes minimum of 12 hours of in-service training a year. Must complete all in-services on time. Completes additional trainings as assigned.
- Other duties as assigned.

Qualifications
QUALIFICATIONS
- Possess a current Maryland CNA certification and the ability to pass a written CNA test required.
- Minimum of one year of experience in a health care setting.
- Basic ability to read, write and understand English.
- Must be able to perform the essential functions of the position with/without reasonable accommodation.
- Ability to recognize safety issues to self and others and be able to assess and implement use of lifts or other personnel to lift and move patients of all sizes and weights in a safe manner.
- Ability to perform consistently in stressful situations.
- Basic computer skills, to include but not limited to Microsoft Word and Outlook, required.
- Effective oral, written and interpersonal communication skills.
- Effective problem-solving skills with the ability to prioritize and delegate.
- Demonstrated commitment to quality, timely and cost-effective patient care.
- Ability to simultaneously handle multiple assignments and projects with attention to details, efficiency, accuracy and deadlines.

The Chesapeake Life Center at Hospice of the Chesapeake provides individual grief counseling and support groups for a variety of populations including adults who have lost a loved one, children and teens, and survivors of homicide and suicide. Through Chesapeake Life Center, we operate two weekend camps for grieving children and teens. We are unique in that we provide grief support services to anyone in the community seeking support including those who have experienced a traumatic or sudden loss. The scope of services far exceeds that which is provided for in the Medicare Hospice Benefit.
