Hospice House (IPU) Director

What You Must Have:

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State license as a Registered Nurse
  • Current State Drivers License
  • Two years of experience as a Registered Nurse in a skilled nursing setting, inpatient care center preferred
  • Demonstrated ability to supervise
  • Nursing skills as defined as generally accepted standards of practice
  • Strong critical thinking, organizational, interpersonal, and communication skills
  • Must be highly motivated, self-directed, flexible, and able to work well and participate in a multidisciplinary group setting
  • Possess good organization, people and problem solving skills and the ability to multi-task many projects and strategies simultaneously

What You Will Do

  • Responsible for interdisciplinary involvement in the development of plans of care to meet each resident’s needs
  • Review patient records to analyze the effectiveness and efficiency of the care provided by the unit
  • Communicate and coordinate with physicians, discharge planners, or appropriate facility representatives from healthcare facilities regarding resident admissions, transfers and discharges as needed
  • Review any/all regulatory reports (2567, Quality Monitoring reports, Quality Indicators, etc)
  • Oversight of onsite pharmacy and drug reconciliation
  • Oversee, direct and coordinate nursing staff; assumes responsibility for continuity, quality, and safety of service of care delivered in compliance with State and federal regulations
  • Promote the philosophy and administrative policies of the Agency, in an effort to ensure quality of patient care
  • Supervise and provide direction to nursing staff and team members, in an effort to ensure quality, compliance with Plan of Care, assessment and reassessment of patient's needs and continuity of services by appropriate health care personnel
  • Supports employee and patient education to enhance knowledge, skills and achieve quality experiences. Continuously trains nursing staff to retain employees with the highest quality of patient care
  • Provide coaching, development, feedback and annual evaluations to team members in a professional and timely manner
  • Monitor QA/PI Program and assures appropriate corrective measures are performed
