Position Purpose:

Provides personal care to patientsreceiving services byMercyOneNorth Iowa Hospice

What you will do:

  • Effective oral and written communication includingtimelydocumentation with all interdisciplinary team members and reports significant changes inpatient’scondition to registerednurse

  • Completion ofcare plan documentationin a timely manner

  • Provides temporary relief topatient’sfamily when in the home setting/nursing facility

  • Participates in regularly scheduled meetings and in-services andmaintainscompetencies.

  • Collaborates with peers to complete weekly/monthly scheduling andfollowsguidelines.

  • Organizes and prioritizes workload within assigned shift/caseload; including the ability todemonstrateflexibility

  • Demonstrate theability to work with frequent interruptionsin order tomeet patient/family needs acrossMercyOneNorth IowaHospice.

  • Utilizes Hospice resources efficiently.

  • Maintains required department confidentialityand abides by HIPPA regulations.

  • Provides competent patient care to the aboveindicatedpatient populations by obtaining and appropriately interpreting data, makingappropriate decisionsbased ondemonstratedknowledge of age-specific growth and development, and appropriately adapting procedures and treatments.

  • Takes an active role in enhancing ability to carry out job functions through personal and professional/job-related growth anddevelopment andparticipatesin medical center and/or departmental education programs.

  • PRN

Minimum Qualifications:

Education:

  • Certification from a Certified Nurse’s Aide (CNA) program through classroom and supervised practical training totaling at least75 hourswith 16 hours devoted to supervised practical training,musthave a minimum of 12 hours of in-service per year tomaintainCNA Certification

OR

  • Currently enrolled in an ADN, BSN, diploma program with successful completion of first semester of ADN program, OR first year completion of nursing courses in BSN or Diploma program, OR graduate of an ADN, Diploma,BSNor LPN program.

  • Must pass Hospice competency requirements Supervised by a licensed nurse.

Experience:

  • Aide experiencestronglypreferred.

Special Skills and Competencies:

  • Ability to successfully complete competency testing

  • Ability to read, write and carry out directions and accept supervisionrequired

  • Empathetic attitude toward the care of the sick and dying and theirfamilies.

  • Demonstrates a commitment to individualdevelopment.

  • Ability to workindependently yetwithin a teamstructure.

  • Competent or willing to learn and develop competence with computer skills and programs and accessin a timelymanner

  • Ability to be flexible, do a variety of duties andto prioritize withfrequentinterruptions.

  • Must be able to read,writeand/or speak English.
