We are currently seeking an experienced Licensed Hospice Home Health Aide (HHA) to join our growing Hospice team serving patients throughout the San Francisco Peninsula

As a Hospice Home Health Aide, you will provide compassionate personal care and support to hospice patients in their homes under the supervision of a Registered Nurse. You will play an essential role in helping patients maintain comfort, dignity, and quality of life while working closely with our clinical and administrative teams to ensure coordinated, high-quality care.

Responsibilities

  • Provide personal care including bathing, grooming, dressing, toileting, and oral hygiene
  • Assist patients with ambulation, transfers, positioning, and range-of-motion exercises
  • Assist with feeding and hydration as outlined in the care plan
  • Observe and report changes in the patient's physical or emotional condition to the supervising nurse
  • Measure and document vital signs as assigned
  • Perform skin care to help prevent pressure injuries
  • Maintain a safe and clean patient environment
  • Follow infection prevention and safety protocols
  • Accurately document patient care provided
  • Maintain patient confidentiality and uphold HIPAA standards
  • Collaborate with the interdisciplinary hospice team to ensure continuity of care

Qualifications

  • Current California Home Health Aide (HHA) Certificate
  • Minimum 1 year of Home Health Aide or caregiving experience (Hospice experience preferred)
  • Current CPR and First Aid Certification
  • Valid California Driver's License
  • Reliable vehicle with current auto insurance
  • Ability to travel throughout the San Francisco Peninsula (approximately 15–25 mile service radius)
  • Strong communication and interpersonal skills
  • Compassionate, dependable, and patient-centered attitude
  • Ability to work independently and as part of an interdisciplinary team
  • High School Diploma or equivalent
